Consideration of Change Order #5 – Rathje Construction, Original Town Urbanization Project – Motion to Approve

It was requested that before paving is installed on Main Street & 3rd Avenue of the Tiffin Phase 3 Urbanization Project to install a 6" water service, 8" sanitary sewer service, 8" ADS storm sewer and a 16" RCP storm line would need to be stubbed out from under the paving to right of way for the future use of the empty lot of 501 Main Street. The 6" water service includes shutting down the existing watermain, cutting in an 8x8 Tee into the existing 8" watermain, installing a 6" valve, laying roughly 30' of 6" C-900 and installing a blowoff with thrust block for flushing and testing per SUDAS Specification 5030 testing and disinfection. 2. The 8" ADS HDPE includes cutting a new hole in the back of intake I-40, laying 20' of 8" HDPE and installing a concrete collar. 3. Other 2 items will be covered under original bid items.
