CHANGE ORDER DATE: 4 February, 2022 Change Order No: 14 (fourteen) Agreement Date: 5 May, 2020 PROJECT: Original Town Urbanization Project: Phase Two HFC#12218.13123 OWNER: City of Tiffin, IA CONTRACTOR: Rathje Construction DESCRIPTION OF CHANGES TO CONTRACT DOCUMENTS: The Fire Department requested the relocation of the street light pole so it is not damaged by Fire Department equipment when leaving for an emergency call. The Fire Department also requested a thicker driveway slab with staged construction which incurred additional costs to the contractor. Cold weather protection of the concrete is also included in this change order. See attached COR #15 for additional information.
