Application for One-Time Water and Sewer Adjustment Name: Mikayla Buchanan Date: 12/19/2023 Service Address: 461 Iris Court, Tiffin, Iowa, 52340 Daytime Phone Number: 319-720-0154 Resident Account Number: 01-01004560-08 Reason for application: The handle on the toilet in my second bathroom stopped working, which caused the toilet not to flush and the water to run continuously. I was not knowledgeable on the topic so I had to call my uncle who is a handy man come check it out. I couldn't have him come immediately because I was unable to take time off from work; so he wasn't able to come until my day off. He was able to determine that the issue was caused by the toilet chain becoming disconnected from the flush valve and fixed it on the spot. I only budgeted $85 for this month's bill as I had no idea that my toilet would have caused the increase, so I ask that you please approve this one time adjustment. I am moving from Tiffin to a new city on January 2nd and I have just enough to make it happen. I plan on paying my last bill and then stopping my services before I move. Thank you for your consideration.
