CHANGE ORDER REQUEST DATE: 11/5/2024 CONTRACT # 0 CHANGE ORDER REQUEST NO. 2 PROJECT: Tiffin Phase 3 Reconstruction Project Send with pay app. CONTRACTOR: Rathje Construction CONTRACTOR PROJECT MANAGER: Dan Scofield CITY OF TIFFIN CONSTRUCTION PROJECT MANAGER: Ben Carnoff DESCRIPTION OF REQUEST: 1. After trees were removed per plan, it was requested that we remove the 2 large trees @ 240 Third Street and grind the stumps. Remove 2 smaller ash trees @ 141 E. 3rd Street. Stumps were pulled and hauled away due to conflict with the proposed sidewalk wall. At 300 E Main Street a large evergreen, multiple saplings and bushes were removed and trimmed branches from the large Aspen tree that would overhang the proposed sidewalk. Stump was ground on the Evergreen and the rest were pulled and hauled away to be processed. 2. Fire Hydrant on the NE corner of Third & Summerhayes was installed to grade shown on staking. When curb was installed the hydrant was 6" to low and has to be raised to match the sidewalk. 3. The home owner at 107 Third Street requested some additional concrete removal and replacement (8'x9') pad and stub the roof drain tile under the slab prior to replacement. After talking to Doug Bolt it was approved to run this additional work through the contract and the city would get with the homeowner for payment. 4. Due to dirt backfill pressure concerns against the wall, it was decided to backfill the Combination Retaining wall with all drainable backfill and filter fabric the entire envelope to keep the dirt out of the rock to keep the water pressure draining through the 4" tile to the storm sewer or through weep holes in the wall. The drainable backfill was left down 8" from the top of wall for topsoil. Rock tickets attached. Plans called for 22 tons, an additional 121.41 tons was placed. This included all material, equipment and labor to install. Work has been completed.
