A signed copy of the Memorandum of Understanding is enclosed for your records. Each agency that participates in the Offset Program is assigned an agency number and org code. The City of Tiffin has been assigned agency number 720 and org code 490. These numbers are used to identify and maintain your records within the Offset Program. To complete your enrollment in the Offset Program please email the following information to us at [email protected]: • A name, telephone number, and email address of a contact person that we can refer your debtors to when they have questions regarding an offset. • Your fax machine # for us to fax your tax matches to. • A completed W-9 to add you to our vendor system. After sending this information you may then log into our Vendor Offset System at https://das-sae.iowa.gov/offsets/. The Vendor Offset System is for internal offset use only and is not to be given out to the public. Here you can maintain your debtor file manually and monitor/process vendor payments matching to your debtor file.
