A - Description of change to be made: 0090 - Decrease Item for PAVED SHOULDER, HOT MIX ASPHALT MIXTURE, 6 IN. 0100 - Increase Item for PAVED SHOULDER, HOT MIX ASPHALT MIXTURE, 8 IN. 0120 - Increase Item for PAVEMENT SCARIFICATION 0130 - Decrease item for STANDARD OR SLIP FORM PORTLAND CEMENT CONCRETE PAVEMENT, CLASS C, CLASS 3 DURABILITY, 10 IN. 0230 - Increase Item for DETOUR PAVEMENT 0240 - Increase Item for TEMPORARY PAVEMENT 8006 - Add Item for DISINCENTIVE FOR BUMPS/DIPS NOT CORRECTED 8007 - Add Item for (PRICE ADJUST) PCC TRANSVERSE GROOVE DEFICIENT 8008 - Add Item for PAYMENT ADJUSTMENT INCENTIVE/DISINCENTIVE FOR PCC PAVEMENT THICKNESS (BY SCHEDULE) 8998 - (PRICE ADJUST) LIQUIDATED DAMAGES B - Reason for change: 0090 - Item is being deleted from the Contract, see Item 0100 0100 - All shoulders were installed at 8-inch thickness and used as temporary traffic surface. The additional thickness allows for better load support for traffic. 0120 - Due to conflicting subcontractor schedules, the contractor performed all scarification at the same time which required additional scarification to produce a 'smooth' (no abrupt elevation changes) pavement surface. 0130 - Item reduced to reconcile under-run of contract quantity. 0230 - Error in estimated quantity for work associated with 2019 winter shutdown. 0240 - Error in estimated quantity for work associated with 2019 winter shutdown. 8006 - See Non-Compliance 003 for Smoothness. One dip was not corrected. 8007 - See Non-Compliance 003 for macrotexture. Areas of the pavement had excessive depth of macrotexture. 8008 - Core test results indicate incentive is warranted. 8998 - Contractor overran the number of days specified in the Contract. C - Settlement for cost(s) of change as follows with items addressed in Sections F and/or G: 0090, 0100, 0120, 0130, 0230, 0240 Contract Unit Prices 8006-8008 - Unit price determined by schedule 8998 - Unit Price established by Contract D - Justification for cost(s) (See I.M. 6.000, Attachment D, Chapter 2.36, for acceptable justification): 8006 - per Standard Specification 2316.05 8007 - per Construction Manual Chapter 2 Appendix 2-34(E). A 5% price adjustment is to be applied. 8008 - per Standard Specification 2301.05A 8998 - per Standard Specification 1108.08 and Page 2 of 4 of the Contract.
