AMENDMENT TO AGREEMENT between SHIVE-HATTERY, INC. AND THE CLIENT PROJECT NAME: Tiffin Public Works Facility CLIENT NAME: City of Tiffin, IA AMENDMENT NO.: 2 ORIGINAL AGREEMENT DATE: June 19, 2020 AMENDMENT DATE: March 30, 2022 City of Tiffin and Shive-Hattery, Inc. (S-H) agree to amend the Original Agreement as follows: A. PROJECT DESCRIPTION: NO CHANGE B. SCOPE OF SERVICES: 1. Add construction testing services for building foundations and structural subgrade (previously scope included only civil site pavement and subgrade). C. CLIENT RESPONSIBILITIES: NO CHANGE D. SCHEDULE: 2022 Construction Schedule E. COMPENSATION: Revise the Estimated Proposed fee for Construction Materials Testing to $8,500. Shive-Hattery will continue to provide when requested by the contractor or the City on an hourly fee basis. F. EXHIBITS: NO CHANGE. G.
