January 8, 2025 CHANGE ORDER NO. 4 WELL 5 PUMP, CONTROLS AND WATER MAIN TIFFIN, IOWA Change Order No. 4 adjusts the final project cost due to additional costs incurred by the City as a result of the issues with disinfection of the raw water line that ultimately led to replacement of the raw water line. The items will be deducted from the final cost and are as follows: 1. Additional cost for water sample delivery and testing. LS $ 2,755.00 2. Additional electrical cost to operate well to provide water to flush main and obtain samples prior to acceptance. LS $4,036.40 3. Additional cost to provide emergency repairs over standard rates for well 4 due to unavailability of Well 5. LS $ 5000.00 4.
