Application for One-Time Water and Sewer Adjustment Name: Jacqueline Astor Date: 12/20/21 Service Address: 655 Kimberlite St Daytime Phone Number: [REDACTED] Resident Account Number: [REDACTED] Reason for application: Bill was 4 times higher than normal. No running water observed throughout entire month. Possible the last couple days as I was out of town at a Christmas party. A plumber is going to inspect the toilet and resolve any potential issue. The normal bill amount is within my budget. Its just that for a couple of days of issue I don't believe that to be accurate but also I make every effort to make timely payments for my normal usage. This is an extreme circumstance and had you had a procedure in place to warn me sooner, I could've looked into any issue earlier within my financial means to have the issue fixed and payment could've been made at that point.
