EXHIBIT "A" Rolling Hills Final Phase I & II 12/15/03 1. Debris around sanitary sewer installation on Joe Johnston's Property. 2. Fencing around concrete dam structure. 3. Pay all subdivision inspection fees to city. 4. Adjust sanitary manholes on the northeast corner of the intersection of Robert's Ferry Road and Goldfinch Drive, to match final grade. 5. Install an additional length of pipe on each side of Goldfinch Drive and provide flared end sections on each end of the existing 24" R.C.P. storm sewer pipe at the Northeast corner of the intersection of Robert's Ferry Road and Goldfinch Drive, and place rip-rap. 6. Place rip-rap between the outlet of the existing 24" R.C.P. that runs east of and parallel to Robert's Ferry Road and the outlet of the 48" R.C.P. that crosses Robert's Ferry Road. 7. Repair and place erosion control fence around sloped disturbed areas. 8. Provide a copy of the paperwork showing that the bacteria testing passed. 9. Reimburse the City of Tiffin for street signs that have been installed on Phase I, and for those that will be installed on Phase II. 10. Clear debris and unused materials from site. 11. Repair leaning light poles. 12. Pour concrete for all intersection radius' in Phase II. 13. Build intercom and pole. 14. Seed all lots in Phase II. 15. Regrade around the pond in the area of Lot 13 and Lot 17 to make it possible for future trail installation. There should be at least 25 feet of property between the property line and the permanent pool level of the pond. 16. Outlets "A" and "B" will be stabilized and established with grass vegetation in a condition that will be ready to mow. 17. Although rip-rap has been placed on the dam structure side of the pond additional rip-rap will be required to prevent wind erosion and to ensure bank stabilization. It is estimated that an additional 10 trucks of rip-rap will be needed. 18. Seal all joints in Phase II. 19. Backfill curb and gutter in Phase II. 20. Utility installation in Phase II to include gas, electric, telephone, cable, and light poles. 21. Place rip-rap, slit fence and grass coverings at all storm sewer outlets. 22. Finish concrete at Intersection of Power Street and Rear Drive. 23. Storm Sewer System. Rolling Hills Phase I & Phase II
