AGENDA ITEM # E-6 AGENDA INFORMATION TIFFIN CITY COUNCIL COMMUNICATION DATE: July 1, 2026 AGENDA ITEM: Approval of Pay Application #4 – Cornerstone Excavating – Forevergreen Road Earth Stockpile Project ACTION: Motion Background The Forevergreen Road Earth Stockpile Project provides for the placement, grading, and stabilization of excess earth materials on City-owned property adjacent to Forevergreen Road. The project includes erosion control measures and site stabilization to properly manage excess excavation material from City construction projects. Cornerstone Excavating, Inc. has submitted Pay Application No. 4 for work completed through June 26, 2026. The application has been reviewed by the project architect and certified for payment in accordance with the contract documents. The work included in this pay application primarily consists of additional erosion control and site stabilization activities completed under approved change orders, including installation of silt fence, mulching and seeding, inspections, bulldozer regrading, and reinstallation of silt fence as needed. These improvements are intended to stabilize the site and ensure compliance with erosion and sediment control requirements. The project architect has reviewed the contractor's application and certified that the work has been completed in accordance with the contract documents. Approval of this payment will compensate the contractor for completed work while maintaining the contractual retainage until final completion and acceptance of the project. Recommendation Staff recommends approval of Pay Application No. 4 to Cornerstone Excavating, Inc. in the amount of $15,004.68. ATTACHMENTS: Pay Application 1
